What is a Primary Care Network?
Additional Roles Reimbursement Scheme (ARRS)
Primary Care Networks have been given funding to provide additional roles to create bespoke multi-disciplinary teams within their individual network.
PCN’s assess the needs of their local population and, working with local community services, make support available to people where it is most needed.
Pendle East PCN
Pendle East Primary Care Network (PE PCN) is a collaboration of five GP practices in Pendle, collectively serving a population of approximately 48,000 patients. You can find details of the member practices here.
Each practice is committed to delivering high-quality, patient-centred care. By working together as a Primary Care Network, we are able to share resources, expertise, and best practices, ensuring patients receive the right care, at the right time, and in the right place.
Our two core priorities are:

We strive to maintain high standards of care and actively listen to feedback from both patients and staff. Our goal is to continue delivering local healthcare services that meet the evolving needs of our community.

ARRS GPs provide general medical care as part of the wider team. They assess, diagnose, and treat patients, provide prescriptions and referrals, and help manage both short-term and ongoing health needs.

A Care Coordinator is a trained healthcare professional who helps to manage the patient’s condition and oversees the patient’s treatment plan. They monitor and coordinate treatment plans with patients in order to educate them about their illness and connect them with various medical professionals as needed.

General Practice Assistants help GPs by managing paperwork, updating records, and supporting simple clinical tasks such as taking observations or arranging follow-up appointments. This frees up GPs to focus more time on patient care.

Advance Mental Health Practitioner
The Mental Health Practitioner works within the Integrated Neighbourhood Team and PCN, supporting people with long-term or complex health conditions that affect mental wellbeing. They provide psychological and emotional support to housebound individuals, often in collaboration with District Nurses, Therapists, Social Services, and community organisations. The role also involves assessing and supporting adults (18+) for anxiety, depression, and other mental health issues, while offering specialist input for older adults, particularly those experiencing memory loss or cognitive impairment.

Psychologically informed wellbeing interventions for those with mental health difficulties. Who support people with strategies and techniques to help them to manage: sleep, depression, anxiety, emotion regulation, stress, confidence building and problem solving.

Pharmacy Technicians support the safe use of medicines across the practice. They help manage repeat prescriptions, assist with medication queries, and work alongside Clinical Pharmacists and GPs to make sure patients receive the correct medication at the right time.

Link Workers help you access non-medical support in the community. They listen to what matters to you and connect you to services like local groups, housing advice, exercise classes, or help with money and wellbeing.
