At Pendle East PCN we provide a range of services to help our practices and patients.

Integrated Neighbourhood Team

The Integrated Neighbourhood Team (INT) has been in place for some time, but we’ve strengthened it by adding new roles to provide even better support for our patients.

Our aim is to improve the coordination between different health and social care services to make it easier for patients to use and ensure that patients only need to tell their story once.

To help you, we have set up a way of working called the Integrated Neighbourhood Team (INT). By doing this we aim to improve coordination and communication between health (including GPs), social care and other partners and you, your family and carers.  We also aim to provide the services you need in your community when you need them.

To support this way of working, your GP has been asked to share access to your medical records when appropriate with health and social care organisations within your local neighbourhood team.  We do this to help ensure that patients get the right treatment and services that they need.

The members of the local health and social care services referred to as the Integrated Neighbourhood Team can include:

  • Complex Case Management Lead 
  • Local Primary Care Team including GPs and Practice Nurses
  • Nursing & Therapy services
  • Social Workers
  • Access to Mental Health Practitioner
  • Intensive Home Support Services
  • Social Prescribers

Our local INT works in partnership with Pendle East PCN, who work closely with other organisations across East Lancashire, so that patients get the right care and support, based on their needs and what matters to them

Social Prescribing – Adults, Children & Families

Social prescribing is a way of improving your health and wellbeing by connecting you to local, non-medical support.

Not all problems need a medical solution. You might be feeling low, lonely, or overwhelmed. You may need help with housing, finances, relationships, or simply finding ways to feel more confident and connected.

Our social prescribing team takes the time to listen and help you find the right support, services, or activities based on what matters to you.

At Pendle East PCN, we have dedicated link workers for:

  • Adults who want to improve their wellbeing or get help with practical issues
  • Children, young people, and families who may need emotional, social, or practical support in everyday life

You can ask your GP practice for an appointment with one of our social prescribing team.

You can also Self Refer to the Adult Service ONLY using the Joy service – Joy – The right support is just a few clicks away. This link is NOT for children and young people.

Mental Health & Wellbeing Support

Looking after your mental health is just as important as caring for your physical health. If you are feeling low, anxious, or struggling to cope, support is available through your GP practice.

Pendle East PCN has experienced mental health practitioners based within the team. They support adults who need more help than talking therapies can provide, but who do not require ongoing care from specialist mental health services.

These practitioners can:

  • Help you understand and manage how you are feeling
  • Offer practical tools and support tailored to your needs
  • Work closely with your GP and other services to join up your care
  • Refer you to other local or specialist services if needed

This service is part of a wider plan to make mental health support more accessible in the community. The practitioners are fully integrated into the GP team and are there to offer timely support in a familiar setting.

Enhanced Access Appointments

Enhanced Access Services are available across all five of our GP practices, offering appointments outside the standard hours of 8:00 AM to 6:30 PM, Monday to Friday. These extended hours provide patients with greater flexibility to see healthcare professionals during evenings and weekends.

Appointment Availability:

  • Monday, Wednesday & Friday evenings (after 6:30 PM):
    The Colne Health Centre
  • Tuesday & Thursday evenings (after 6:30 PM):
    Yarnspinners, Nelson
  • Saturdays:
    Yarnspinners, Nelson
    • 9:00 AM – 1:00 PM: Face-to-face appointments
    • 1:30 PM – 5:30 PM: Remote telephone appointments

Services Offered:

  • Blood tests (paediatric and adult)
  • Cervical smears
  • Annual health reviews
  • Appointments with GPs, Advanced Nurse Practitioners (ANPs), Health Care Assistants (HCAs – including paediatrics), and Nurses (offered both remotely and face-to-face)

Appointments can be booked directly through your usual GP practice.

Health Coaching

Health coaching is a free service that helps you build the motivation, confidence, and skills to make lasting improvements to your health and wellbeing.

Rather than giving you advice, a health coach works with you over a series of sessions (usually around six) to help you set realistic goals and create healthier routines that suit your life.

You might want support with:

  • Becoming more active
  • Reducing stress or low mood
  • Eating more healthily
  • Losing weight in a sustainable way
  • Managing a long-term condition like diabetes or arthritis

The service is delivered by Adrian Lamb and is part of the Pendle East Neighbourhood Team, based at Colne Health Centre. It’s available to patients registered with:

  • Harambee Surgery
  • Barnoldswick Medical Practice
  • Pendle Medical Practice
  • Richmond Hill Practice
  • Barrowford Surgery
